TERMS and CONDITIONS

This is an Agreement between Happy Campers Pet Lodge (previously Kottage Kennels & Suites) and the Pet (s) Owner whose signature appears below, called "Owner. " This agreement contains the entire agreement between the parties. All terms and conditions of this agreement shall be binding on the heirs, administrators, personal representatives, and assigns of the Owner and Happy Campers Pet Lodge (previously Kottage Kennels & Suites). The Owner acknowledges that the terms and conditions set forth are subject to change and agrees to the terms and conditions presented upon the service date.

1. Pet CARE: Happy Campers Pet Lodge shall exercise reasonable care and keep the kennel premises sanitary and adequately enclosed for the Pet (s) delivered by the Owner to the pet care provider. The pet guest(s) must be fed regularly and enough and housed in clean, safe quarters. All pet guests boarded, trained, groomed, transported, or otherwise cared for by Happy Campers Pet Lodge without liability on Happy Campers Pet Lodge's part for loss or damage from disease, death, running away, theft, fire, injury to persons, other pets, or property by said Pet, or other unavoidable causes, due diligence and care having been exercised.
2. Pet BEHAVIOR: Although Happy Campers Pet Lodge supervises the dogs carefully and does not take aggressive dogs, dogs will be allowed to play together, and the Owner acknowledges that it may be hazardous. Owner agrees. Owner is solely responsible for any harm caused by their Pet (s) while Pet (s) is attending Happy Campers Pet Lodge and is solely responsible for any and all acts or behavior of said Pet while it is in the care of the pet care provider, to include payment of costs for injury to staff, pet guests or damage to facilities caused by the Pet (s). The owner agrees that if Pet (s) causes damage, it will be at the pet care providers' discretion to relocate the Pet (s) to another accommodation or boarding facility. The owner further understands that they will be responsible for paying the initial agreed-upon rate of stay. Owner releases Happy Campers Pet Lodge, its Owner, employees, and volunteers of any liability arising from their pet(s) attendance and participation. Owner specifically represents that Pet (s) have not harmed or shown aggression or threatening behavior towards any person or any other dog.
3. Pet HEALTH: The owner explicitly represents to Happy Campers Pet Lodge that, to Owner's knowledge, the Pet is in good health and has not been ill or exposed to any contagious diseases within thirty days before check-in. During the period of this Agreement, Owner also agrees to notify Happy Campers Pet Lodge of any known exposure of the Pet to infectious disease and hold the Pet out of attending Happy Campers Pet Lodge until the Pet (s) is symptom-free for a minimum of 15 days or with written veterinary clearance. The Owner acknowledges that Pets not spayed risk coming in-season during their stay, and Happy Campers Pet Lodge reserves the right to send said Pet home if necessary. The Owner acknowledges that any expenses incurred due to the Pet being in-season will be at the Owner's expense and the total cost of boarding charges. Happy Campers Pet Lodge reserves the right not to board, or groom said Pet who arrives with a possible contagious health issue. Pets arriving ill, have diarrhea, are in-season, pregnant, or nursing, the Owner agrees to pay all boarding fees and cancellation fees that may apply due to the cancellation of the reserved reservation.
4. ELDERLY PETS & HEALTH: The Owner agrees and acknowledges that boarding, daycare, or grooming of an elderly pet or Pet with health problems is at the Owners own risk and may cause the Pet additional stress in which the Pet could become ill or result in death. Owner releases Happy Campers Pet Lodge, its Owner, employees, and volunteers of any liability arising from their Pet (s) attendance.
5. VACCINATIONS: The Owner authorizes Happy Campers Pet Lodge to contact their veterinarian to confirm health, temperament, and vaccination records. The Owner further agrees to maintain the currency of vaccinations and provide documents from the family veterinarian as Happy Campers Pet Lodge policy requires.
6. SPECIAL SERVICES: The Owner further agrees to pay all costs and charges for special services requested and all veterinary costs for the Pet (s) during the period said Pet (s) is in the care of Happy Campers Pet Lodge.
7. FLEAS/TICKS: The Owner further agrees to be financially responsible for any required treatment for fleas/ticks if Happy Campers Pet Lodge determines to provide a flea/tick-free environment. If fleas or ticks are abundant, Happy Campers Pet Lodge reserves the right to refuse to the board said Pet, and all cancellation terms and fees will apply.
8. GROOMING: The Owner authorizes the groomer to de-matting/thin out matted hair if required during the grooming process and that it may cause patchiness and hair loss. The Owner also acknowledges that shaving/stripping of Pet (s) will dramatically change the Pet's appearance, and the hair will be very close to the skin and may expose pre-existing skin conditions or permanent hair loss. Grooming may expose pre-existing health/skin problems for which Happy Campers Pet Lodge shall not be held liable.
9. Pet EXPOSURE: The Owner understands that there is a possibility for injuries, infectious diseases, and parasites when dogs play or are boarded together.
10. OFF PREMISE:  Happy Campers Pet Lodge agrees that the pet guest(s) will not be taken off-premise except for veterinary care or by the Owner's consent.
11. Pet TRANSPORTATION: The Owner requesting pick-up, delivery, or pet taxi services authorizes Happy Campers Pet Lodge to transport their Pet (s). Owner releases Happy Campers Pet Lodge, its Owner, employees, and volunteers of any liability arising from their Pet (s) transportation.
12. EMERGENCY: The Owner understands and authorizes Happy Campers Pet Lodge that if Pet (s) becomes ill or injured, the Owner shall be notified at once, and if the Owner does not inform immediately regarding measures to be taken or if the state of the animal's health demands quick action and otherwise requires professional attention, Happy Campers Pet Lodge, in its sole discretion, may engage the services of a veterinarian or administer medicine or give other requisite attention to the animal. The Owner agrees to be solely responsible for paying all medical bills and releasing Happy Campers Pet Lodge, its Owner, employees, and volunteers of any responsibility related to such medical care, and the expenses thereof shall be paid by the Owner. This care may be given under whatever conditions are necessary to preserve the life, limb, or well-being of pet guest(s).
13. RATES: The owner acknowledges that boarding, grooming, and training rates are subject to change and agrees to pay the rate for pet care provided in effect on the date pet(s) are checked into Happy Campers Pet Lodge. Advanced deposits will be required for all major holidays and boarding reservations made months in advance. Special rates will apply for extra-large pets, the elderly, and special needs. Crated pets will have an additional daily charge for evening turn-out.
14. CANCELLATIONS: The Owner agrees to make any cancellations during regular business hours Monday-Friday. If Happy Campers Pet Lodge is notified of a cancellation after hours or on a Saturday or Sunday, the cancellation request will be reflected on the next weekday. Happy Campers Pet Lodge must be notified by TELEPHONE or EMAIL to ensure notification since a cancellation email will be provided. Happy Campers Pet Lodge will not accept any voicemail cancellations. If Happy Campers Pet Lodge is notified less than the required timeframe, charges will be placed to the Owner's credit card on file for Non-Holiday or Holidays. If the Owner calls to cancel a reservation on the scheduled arrival date of the reservation or any subsequent day or does not show up on the reserved days, the Owner will be responsible for the full cost of the time booked. Advanced deposits will only be refunded if the reservation is canceled within the appropriate time frame designated during NON-HOLIDAY or HOLIDAY REQUIREMENTS. There may be additional cancellation penalties for reservations made three months or more in advance or during higher demand periods. If the Owner shortens their trip (EARLY CHECKOUT) during HOLIDAYS or FULLY BOOKED peak vacations while the Pet is being boarded with Happy Campers Pet Lodge, thereby canceling reserved days, the Owner will be responsible for the total cost of the number of days reserved. 
15. OWNERSHIP: The Owner explicitly represents that they are the sole owner of the Pet (s), free and clear of all liens and encumbrances, and the Owner is at least 18.
16. PAYMENT: The Owner agrees that all charges will be paid in full before the Pet (s) leaves Happy Campers Pet Lodge. The Owner further agrees that the Pet (s) shall only leave the facility once the Owner pays all amounts due or proper payment arrangements are agreed upon by both parties. The Owner acknowledges that Happy Campers Pet Lodge is under no duty to make payment arrangements.
17. PICK-UP: The Owner agrees to collect the Pet (s) at the time stated after boarding, training, or grooming, or the Owner will be charged boarding fees for each day uncollected. Owner agrees that all charges incurred by the Owner for boarding, grooming, training, medicine, or veterinary services shall be payable upon pick-up of Pet (s) or if Pet (s) are not called for within 10 days after the time for the return of Pet (s), the Pet (s) will be sold within 10 days after this period, Happy Campers Pet Lodge shall have and is hereby granted, a lien on the Pet (s) for all unpaid charges resulting from services provided by Happy Campers Pet Lodge. The Owner now agrees that in the event the costs are not paid when due in accordance with this contract, Happy Campers Pet Lodge may exercise in writing by registered mail its lien rights upon 3 days' written notice before the date of such intended sale to the Owner at the address given hereon, and no further notice shall be deemed necessary. Happy Campers Pet Lodge may dispose of Pet (s) for any unpaid charges, at private or public sale, or otherwise for the best price available in the sole discretion of Happy Campers Pet Lodge, and the Owner specifically waives all statutory or legal rights to the contrary. If the such sale does not secure a price adequate to pay such costs of Pet (s) care or other charges delinquent, plus costs of sale, then the Owner shall be liable to Happy Campers Pet Lodge for the difference. All monies realized by Happy Campers Pet Lodge at such sale, any excess amount over and above the charges due costs of sale, shall be paid to the Owner.

Signature: By signing this agreement and leaving Pet (s) with Happy Campers Pet Lodge. The Owner certifies the accuracy of all information given about said Pet (s). Happy Campers Pet Lodge reserves the right to deny admittance to Owner's Pet (s) for any reason at any time. By signing this form, the Owner certifies that they acknowledge and have read, understood, and accepted the terms and conditions set forth by this agreement and that he/she is the sole owner of the Pet (s) or that he/she is acting on the Owner's behalf, and that he/she is authorized to sign this form. Further, the undersigned is of legal age of majority.
